1. Circus of the Last Days
Upon arriving in Rivington, you'll find a magical circus. It's filled with distractions, but it hides a grim secret.

Dribbles the Clown
Volunteer for Dribbles' stage act. It turns out he's a Doppelganger working for Orin. After defeating the ambush, Necromancer Lucretious will ask you to find the severed body parts of the real Dribbles scattered across the city.

Akabi's Jackpot (Legendary Trident)
Spin the wheel of Akabi the Djinni. He rigs the game using a Mage Hand ring. Pickpocket the ring from him, then spin again to win the jackpot. Angry that you cheated his cheating, he teleports you to a Jurassic jungle. Fight past the dinosaurs to find a chest at the exit containing Nyrulna, a legendary throwing trident.
2. Open Hand Temple Murders
Father Lorgan has been murdered, and the holy elephant investigator Valeria blames a refugee. To enter the Lower City legitimately, you must solve this case.
Enter the basement of the Open Hand Temple through the kitchen. Press the hidden buttons behind the heraldry signs to open a secret cave. Defeat the Doppelgangers inside, then loot the Flower Key. Use the key in Fraygo's Flophouse (third floor wardrobe) to discover the serial killer's list, proving a Bhaalist cult is behind the murders. Show this to Valeria at Sharess' Caress to get a Lower City pass.
3. Ironhand Gnomes & The Gur Camp
If you saved Barcus and Wulbren in Acts 1 and 2, they have a secret base in a cave accessible from the beach under the Rivington bridge.
Wulbren wants to blow up the Steel Watch Foundry using a Runepowder Bomb. Agree to take the bomb. Barcus, however, wants a peaceful transition of power. This sets up a major conflict for the Lower City.
In northeast Rivington, you find the camp of the Gur monster hunters. They are hunting the vampire lord Cazador Szarr because he kidnapped their children.
- If Astarion is in your party, they will recognize him as the one who lured their children away.
- You can promise to help them rescue the children, securing them as powerful allies for the final battle.
- Warning: If you let Astarion ascend later in the Lower City, the Gur will ambush your camp and you will have to slaughter them all.
4. Wyrm's Rock Fortress & The Coronation
Pass the tollgate using your new pass (or fly/jump across the cliffside to bypass it entirely). You are invited to attend Lord Enver Gortash's coronation as Archduke.
Attacking Gortash during the coronation ceremony is essentially suicide. The room is packed with flaming fist guards, Steel Watcher mechs, and Duke Ravengard. Hear him out, agree to an alliance (you can break it later), and leave peacefully to access the Lower City.
5. Secret: The Legend of Ansur
If you rescue Duke Ravengard later, he tells you a dragon sleeps beneath Wyrm's Rock. You can go there right now.
In the Wyrm's Rock Prison, look for a dead end with two dragon-head torches. Shoot them with lightning damage to open the Wyrmway. Pass the four trials of Balduran (Justice, Insight, Strategy, Courage) to enter the dragon's lair.
- You will find Ansur dead, raised as an Undead Dragon. This triggers a massive boss fight. When he charges his supernova attack, take cover behind the crystal formations or use Globe of Invulnerability.
- Defeating him yields the Balduran's Giantslayer (Legendary Greatsword) and the Helm of Balduran (Legendary Medium Armor, heals every turn).
