All 18 trophies and achievements come from the campaign, and none of them can be missed for good: every mission can be replayed from the campaign menu once it is finished. Several need a higher difficulty only for one stretch of a mission, and the difficulty can be changed from the pause menu just before that stretch. The difficulties are Recruit, Regular, Hardened and Veteran from the start; Realism opens after one full run of the campaign, and YOLO after a run on Realism.
Mayhem
Hajin City, South Korea. You play Private Park, a young South Korean soldier in a joint squad of South Korean and US Marines, on patrol as tension rises on the Korean peninsula.
-
1
The convenience store
Follow Pvt. Dunn into a convenience store; he is buying food and stomach medicine for Pvt. Ju Kwon. Dunn asks Park twice for help, about the food and about what he came to buy. Every answer only changes the banter: none of them has a consequence. Pay the clerk at the counter to move on.
-
2
To the stadium
Outside, a cutscene hands you your weapon. Follow the squad and crouch through the debris. A missile barrage starts falling as you head down the streets towards the stadium; stay with the squad, off the street, and go in through the Netshin Wolves store.
-
3
The baseball field
Lt. West takes point and the squad crosses the field to meet Charlie Platoon. This is the first fight: enemy paratroopers drop onto the field. Use the dropped supply packages as cover and watch your armour; you start with two spare armour plates. The Skeet Shooting trophy is earned here (see below).
-
4
The restaurant and the patrols
Follow the squad through the stands into a building with a supply cache of ammunition and armour plates. An enemy patrol drives past: crouch and stay out of sight. Whenever the objective says not to be spotted, being seen ends the mission. Wait for the squad, then leave by the back door. Go prone when a convoy rounds the corner, then hide between the cars in the parking garage while another patrol rolls through. Leaving the mission area, even briefly, sends you back to the last checkpoint.
-
5
The tunnel and the fuel truck
Lower in the garage is the tunnel the convoys came from, and a fight; use the pillars and cars as cover. A fuel truck blocks the tunnel. Hold off the attackers while the squad pushes it downhill, take the resupply cache on the right, then pick up the RPG on the left and blow up the truck. A cutscene and the title sequence end the mission.
Kill 10 paratroopers in Mayhem before they hit the ground. It happens on the stadium field in step 3. Look up as the drop starts and shoot the enemies still in the air before anyone already on the ground. The ten have to come from the same drop: if they all land first, reload the checkpoint and try again. One guide also suggests setting the field of view to 60 for this part, which zooms the view in a little, and switching the primary weapon to single shot.
Reward: finishing Mayhem unlocks the Cho Operator for multiplayer.
Out in the Cold
The story jumps back five days. You play Captain Price, working alone in New York, and his target is Milena Romanova, who is in a nightclub. Most of the mission is close combat, and two of its trophies need a higher difficulty for a single room or a single fight.
-
1
Follow Milena
Work through the crowd and up the stairs on the left, keeping Milena in sight: losing her ends the mission, and the find-objective button puts a marker on her. A drunk patron stops you on the stairs; fight him with your fists and block his blows. At the VIP area, use the rope gate, counter the bouncer when he steps in and take his earpiece, which lets you hear that security plans to jump Price outside. Another guard in the crowd can end the mission as you follow Milena: take him down with a takedown.
-
2
Locate Makarov
Makarov steps into the lift. Fight the two guards at the bar, call the lift, beat the two more inside it and take a handgun. Armed security ambushes you as the doors open. Shooting a civilian ends the mission, so pick your shots and use cover. Some guards carry the VEL 46 SMG, worth picking up. There is one way forward, through rooms of screen art: one of pink flowers, then one of green forest, where the first two guards shoot from behind glass you can shoot through and more come through a door on the left. Take that door into a back corridor; a civilian falls out from behind a vending machine there, so do not fire at her, only at the guard beside her. The corridor leads to the kitchen, another fight (and the Safety’s On trophy, below). Leave the kitchen by the door on the left for a cutscene with Milena.
-
3
The rooftop chase
Follow the service corridor outside into a narrow alley, with enemies on the catwalk above. Climb the ladders, fighting on the way; at the top an enemy takes your gun in a melee. Up the next ladder, Makarov is about to board a helicopter: tackle him at the prompt. The chase then runs across the rooftops and through flats, with jumps, ducks and slides to time; the one that kills you if missed is the jump at the end of the slide down a ramp. The chase ends in an alley at street level.
-
4
The fight with Makarov
The alley fight is melee only, and Makarov has a knife. The rule throughout is to block just before his strike lands, then counter. It comes in two parts: a scripted knife sequence against the metal shutters, then an open fist fight in the alley, where a takedown prompt appears after enough clean parries and counters. Land one or two hits after each parry, not more; a longer combo lets him block and strike back. A cutscene ends the mission. The Can’t Touch This trophy is earned in this fight.
Defeat all enemies in the kitchen of Out in the Cold without firing your gun on Hardened or harder. The kitchen is the room after the forest screen room and the back corridor. Up to that corridor you can shoot freely and play on any difficulty; switch to Hardened or higher just before the kitchen.
One guide’s method: pick up the glass bottles you pass on the way and arrive with four. The kitchen holds five enemies. Switch to your fists, run at the two on the right, throw a bottle at the first and take him down when prompted, then do the same to the second. A third comes at you: bottle and takedown again. Go round the shelf, throw the last bottle at the nearer enemy, and finish the final one with melee.
Defeat Makarov on Veteran difficulty or higher without getting hit in the alley. The rest of the mission can be played on any difficulty: open the pause menu before the alley and switch to Veteran or higher. A hit is the screen flashing red; blocked strikes do not count. The moment you are hit, reload the last checkpoint rather than fighting on. There is a checkpoint partway through the fight, so a hit in the second half only sends you back to that point, and the trophy still unlocks.
Reward: finishing Out in the Cold unlocks the No More Rules calling card.
Chapter Completion
The campaign now follows two stories that meet later: Park’s squad in South Korea and Price’s own hunt abroad. The next mission puts you on a third side of the war.

Cross-checked
Cross-checked on 21 October 2026 against IGN’s walkthroughs of Mayhem and Out in the Cold and its mission list, the trophy guides and lists of PowerPyx, Insider Gaming, GameRant and GAMES.GG, GAMES.GG’s guide to the Makarov fight, the campaign-choice tables of GameRant and Destructoid, Polygon’s and Games Atlas’s mission lists, Insider Gaming’s plot summary, and the reward lists of Insider Gaming, GameRant, Games Atlas and Power Up Gaming. Some of it rests on fewer than three: the step-by-step route of both missions, and the jump back five days, are IGN’s alone; the Skeet Shooting checkpoint reload, the field-of-view tip and the bottle method for Safety’s On are PowerPyx’s alone; the two parts of the Makarov fight and the one-or-two-hits advice are GAMES.GG’s, with IGN describing the knife; the pause-menu switch, the red flash and the checkpoint partway through the fight come from PowerPyx and GAMES.GG. Not from a playthrough: nobody here has played it.
