The choice
After you beat mutated Jack in the boathouse, he comes back once more and you use one dose of serum to finish him. One dose is left, and both Mia and Zoe need it. Whoever you choose leaves with you on the boat. The guides tie both endings to this one decision. Make a manual save before you choose if you want both endings from one run.

Getting both
- Either order works. One trophy guide says neither choice changes much in how the game plays.
- You play as Mia on the wrecked ship in both endings, so the videotape sections and their trophies are the same.
- One trophy guide suggests taking the other ending on the speedrun, so that Madhouse can be played with Mia saved, which skips the extra fight on the ship.
- If you keep a manual save from just before the choice, you can load it after the credits and play from there to the other ending.
- One guide says both endings close with the Baker family photo in the credits and, after the fade to black, a phone vibrating.
