Three endings are decided by behaviour the game never announces, tracked quietly across the whole run. The other five need a second run — three original endings that need items carried through New Game+, and two the remake added on top.
The three that depend on how you play
🚪 Leave
Heal when you take damage, keep your distance from Maria without abandoning her, and return to Mary's photograph and letter through the game.
🌊 In Water
Play through hurt without healing, never examine Mary's letter or photo, and examine Angela's knife when the game offers it.
🌹 Maria
Stay near her and keep her from taking damage while she is with you, then check on her in the hospital and in her Labyrinth cell whenever the game gives you the option.
The three that need a second run
⚱️ Rebirth
Carry four items through New Game+: White Chrism, the Obsidian Goblet, and the "Crimson Ceremony" and "Lost Memories" books.
🐕 Dog
Find the Dog Key on a later run and open the observation room in the hotel.
🛸 UFO
Use the Blue Gem at the places the game marks for it on a later run.
The two the remake added, also on a second run
✨ Bliss
Open the safe in Pete's Bowl-O-Rama with the code 1887 for a key, use it on the garden chest at Brookhaven Hospital for White Claudia, then drink it in Room 312 and play the tape.
🕯️ Stillness
Needs the In Water ending already earned, or the key never appears. Find the Key of Sorrow in the green car on Wiltse Rd, just past the metal gate at the start (break the window with the chainsaw). Then use it with the code 314 on the Manager's Office safe in the Otherworld Lakeview Hotel to get the Toluca Postcard. Examine the postcard in your inventory and finish the game.

Updated on 23 September 2026 against PowerPyx's endings guide and Wikipedia, which added Bliss and Stillness to the eight-ending total, and re-checked on 1 October 2026 against Eurogamer.es, Vandal and EliteGuias. Both need a second run and neither is required for any trophy — the six original endings above are the ones that are. Not from a playthrough: nobody here has played it, and what the sources do not describe is not on this page.
Where the second-run items actually are
The three New Game+ endings are the ones this page can be specific about, because they hang on
objects in fixed places rather than on behaviour the game never shows you.

The Dog key being two parts is the detail that wastes runs. Finding one half and
assuming the key is a single object sends people to the hotel with nothing to open the door. Pick up both in South
Vale before you ever reach Lakeview.
And the honest caveat about the first three: the behaviour-tracked endings
are not perfectly consistent. Following every step for Leave or In Water does not guarantee you get it. That
is the game, not this page — if you need a specific one, keep a save before the final stretch rather than
trusting the checklist.
Still to verify
Cross-checked on 5 September 2026 against PowerPyx and a Game8/Gfinity/GameRant composite — which agree on the Dog key halves (Pet Center, then a Katz Street doghouse), on the Observation Room being on the Lakeview Hotel's 3rd floor, and on the Blue Gem's four spots ending at Room 312. They also corrected two things this page had wrong: the fourth Rebirth item is the "Crimson Ceremony" book, not a "Lapis Eye" — no source names an item by that name — and the Blue Gem's confirmation is a visual glow, not a sound cue, which no source we found describes. What is still unverified is the only thing reading cannot settle — nobody here has played it.
