The premise
A murder he did not commit
The Order's patriarch is killed and Soul is framed for it. That is the event the game opens on and the thread everything else hangs from.
A wound that should have finished him
In the hunt that follows, Soul is gravely wounded — the duel shown in the trailers is with Zuoshang, once his blood brother, and Soul loses it.
A cure worse than the injury
A figure the previews call the Flesh Sculptor, or the mystic healer, saves him — not by healing him but by replacing what was destroyed with a tainted heart. It works. It works for sixty-six days.
And a clock
That is the frame: find who did it before the borrowed heart stops. The hardest difficulty is named after it, which tells you how central S-GAME considers it.
